smaller patient panel sizes to preserve personalized care and access, future waitlists may

Traditional insurance billing (or transparent cash-pay pricing) combined with an annual
membership that unlocks enhanced access, longer visits, and care-coordination benefits.
Monday through Friday: 8:00 AM–5:00 PM. Saturday: 8:00 AM–1:00 PM. Saturday
appointments are reserved primarily for sick visits, well-child visits, pediatric
appointments, sports physicals, and minor urgent concerns. After-hours’ on-call support is
available.
Absolutely. Our goal is to help patients avoid unnecessary urgent care visits whenever appropriate and
access to your provider on weekends by phone when possible and even urgent telehealth
calls. Same day appointments will be scheduled to the best of our abilities as we
understand urgent needs can arise at any time.
Yes. Sunapee Harbor Health will offer telehealth visits when clinically appropriate and not until
after you are established as a patient with an in-office visit. You are REQUIRED to be in the
State of NH for Telehealth services as we are licensed providers in New Hampshire. We will
also offer Telehealth options when appropriate after hours and on weekends to have
members avoid urgent care visits when appropriate.

No. Patients are welcome whether they have insurance or not. We will post a transparent
pricing page for office visits and additional testing such as urinalysis, strep tests, swabs, etc.

Sunapee Harbor Health is focused on preventative and primary care and although we will
suture, perform small office surgical procedures, run rapid tests, we are not going to be
offering hospital-based services. We have a great relationship with local hospitals and
independent specialists and will refer you to where you choose without hassle.
Yes. Athena is our medical records software and has full bi-directional communications with
Epic (numerous local hospitals use this system) as well as Quest Labs and Lab Corp.
No. Medicare patients are exempt from membership fees but are strongly encouraged to
participate in Advanced Primary Care Management (APCM).

Advanced Primary Care Management is a Medicare-supported program that enhances
communication, care coordination, chronic disease management, and ongoing support. This
can mean as frequently as a monthly chart review regarding your health care plan by our
team and reach out when appropriate when we find care deficits or new treatment options
that may benefit the patient. Please see APCM page (page will be updated frequently).
Yes. There needs to be an appropriate reason such as frailty, increased fall risk, danger to
patients because of leaving the home, bed bound status, or other needs that will be
considered on a case-by-case basis. Home visits are something Ben Holobowicz PA has been
doing for decades and plans on continuing, however home visits may be limited by
scheduling so should never stop someone from seeking emergency medical care. Palliative
care and hospice services are also offered.
You and your spouse or domestic partner plus unlimited dependent children 22 and
younger.
Absolutely. We welcome patients of all ages--from newborns through adulthood. We are particularly
excited to welcome back Aram Kalpakgian PA to the building he began his career!
Furthermore, Aram is practicing primary care for all ages, not just his large following of
pediatric patients. All four medical providers see pediatric patients.
Yes. With many years of experience, this is a service we do offer.
Yes, we do. However we are extremely fortunate to have some amazing women’s health
practitioners in the local area and will place referrals to the gynecologist of your because
finding that exact provider that you are comfortable with and stay with them is very
important to your health.
While we will manage chronic, well documented pain, we are not a substance use disorder
clinic, we do not have Licensed Alcohol and Drug Abuse Counselors on staff, nor are we
affiliated with a clinic. If you have substance issues, you will need to contact appropriate
specialty clinics with extensive wrap around services to receive the most appropriate care
possible. Patients who are stable on long-term Suboxone and have therapists for the
necessary support will be considered on a case-by-case basis.
Yes. Membership may be canceled at any time; however, membership fees are non-refundable.
If you chose not to renew your membership, we would honor the patient-provider
relationship until the anniversary date, provided you remain a resident of NH and no
mutual agreement to transfer care elsewhere.
We respectfully request that you arrive 20 minutes before your appointment, especially on
your first visit or when you have new insurance as we have requested updates for your
chart. Arriving late creates a chain event that often affects all the patients after you for the
remainder of the day. We will do our absolute best to make sure we are on time for you so
please do the same for us.
If you arrive 10 minutes after the start of the appointment, we will kindly ask you to
reschedule. We will call or send you an email after your first No-Show as we know life is
unpredictable and you may need to be seen urgently. After, but the second No-Show, a
pattern is being formed, and we may have to ask you to find another health care facility. We
understand true reasons that cannot be planned for and will make exceptions when
appropriate.

One of the missions of Sunapee Harbor Health is to focus on preventative care, which means
longer visits for education, encouragement, and feedback. In today's fast paced medical
system, we want to be able to spend more time with our patients, availability for neededurgent follow up visits AND have rapid access for acute care needs. Limiting our panel size
allows us to provide this great service to our community.
