pricing with guaranteed rate protection for patients who enroll during our launch phases.

Sunapee Harbor Health is intentionally designed around smaller patient panel sizes. By
caring for fewer patients, we can provide greater access, longer appointments when needed,
more personalized care coordination, and stronger relationships with our patients. Because
enrollment is limited, we anticipate that waitlists may develop as the practice grows.
Enrolling during the Founders or Early Member periods not only secures your place within
the practice but also locks in preferred pricing for years to come.
Medicare beneficiaries are always welcome at Sunapee Harbor Health and are not charged
with annual membership fees. Medicare patients are exempt from our membership
program. Instead, we strongly encourage participation in Advanced Primary Care
Management (APCM)*, a Medicare created program designed to improve communication,
care coordination and ongoing proactive healthcare.

Sunapee Harbor Health is proud to care for Medicaid patients residing in the Town of
Sunapee. Eligible Medicaid patients are not charged with annual membership fees.

Monday through Friday: 8:00 AM–5:00 PM. Saturday: 8:00 AM–1:00 PM. Saturday
appointments are reserved primarily for sick visits, well-child visits, pediatric
appointments, sports physicals, and minor urgent concerns. After-hours’ on-call support is
available.
We understand that transitioning to a new healthcare provider can be overwhelming. Our
goal is to make the process as simple and stress free as possible.
Sunapee Harbor Health is currently developing a secure electronic enrollment and medical-
record transfer process using electronic signature technology. Beginning August 31, 2026,
team members will be available in the office to answer questions, provide tours, assist with
registration and enrollment, and coordinate medical record transfers.
